Thank you for filling out this CBC Student Release form. Please fill out this form for all students involved in the CBC Students Ministry. (You will be able to complete this form once for any number of students.) This release form will be valid until the end of the calendar year. Each year this release form will need to be resubmitted for students participating in the CBC Student Ministry.
